Tobacco heires Doris Duke (Susan Sarandon) hires Bernard Lafferty (Ralph Fiennes), an Irish butler to manage her New Jersey home. As time passes they grow close and he begins to travel with her. His role in her life expands. Her lawyers question his motives, but he refuses their payoff. His alcoholism returns and she sends him to rehab. She has a stroke and he nurses her. She dies and names him executor of her personal estate. He dies three years later.
